I keep relapsing after the 1-3 day mark, can anyone help me out with stopping myself?

Dopamine detox. What other things do you have that trigger a dopamine release? Can you slowly wind those down, too? The brain wants what it knows, so you have to retrain it and that can take time. But it does work.

This rewiring takes real life change to truly shake up. On top of what Charlie said; if you play a lot of video games or watch a lot of TV, cut it out completely or almost completely. Get off social media for a few months and limit your screen time to like 2-3 hours a day max. The less electronic entertainment the better. Your mind is not only wired to watch porn, it is wired to find distraction and escape. If you remove these things, you remove comforts and triggers, and you are forced to live in reality which will do you many favors when trying to make significant change to your behavior and thinking.
